Patient B has been diabetic for 10 years.

She has a neuropathy which is stable and may related to treatment for her colon cancer in 2000. She has lost some weight since diagnosis and the weight has been more stable recently. Her glycohemoglobin originally decreased and has been fluctuating for the past two years between 6.9 and 7.6.  The AST increased when the atorvastin was increased.  

If she follows a better diet and sheds a few more pounds, the diabetes should improve.  Additional agents for blood pressure control and possibly  glucose control are in order.

Patient A has been diabetic for over 20 years. The glycohemoglobin, with therapy, fluctuates between 6.9 and 7.6. She initially lost some weight but has since been stable. She has a mild diabetic neuropathy for the past five years. Her creatinine has increased in the past five years, but is still in the normal range. She also has carotid artery stenosis that may have progressed in the past two years. The AST is stable. 

She is unlikely to lose weight. Additional medications can be added to further lower the LDL towards 60.  Better control of her blood sugar requires additional medications, diet and exercise. Her renal problem, while still sub-clinical, needs further evaluation to see if it is from diabetes or another cause that may require different treatment
